Carers Support
Introduction
At Arbury Medical Centre, we recognise the valuable role carers play in supporting family members, friends and neighbours. A carer is anyone who provides unpaid help and support to someone who could not manage without their assistance due to illness, disability, mental health needs, frailty or substance misuse.
Are you a Carer?
You may be a carer if you:
- Help someone with personal care
- Support someone with medication or appointments
- Provide emotional support
- Help with shopping, household tasks or finances
- Look after a relative or friend who could not manage without your help
Many people do not realise they are carers.
Register as a Carer
If you are a carer, please let the practice know.
By registering as a carer, we can:
- Record your caring responsibilities on your medical record
- Offer appropriate support and signposting
- Identify services that may be beneficial to you
- Consider your needs when arranging appointments and services
Please complete the form to register as a carer.
Registering as a carer helps us understand your needs and ensure appropriate support is considered when planning and delivering your care.
